Using personal CGM to improve diabetes control in hospital

  • You are 18 years old or older.
  • You have diabetes or high blood sugar that requires insulin shots or an insulin pump.
  • You are admitted to a non-ICU hospital unit or the Advanced Care at Home program.
  • You expect to stay in the hospital for more than 48 hours.
  • You already use a continuous glucose monitor (CGM) at home and can bring it with you.
