- You are age 18 or older and female
- You had high blood pressure during pregnancy (gestational hypertension or preeclampsia) but not chronic high blood pressure before pregnancy
- Your body mass index (BMI) was 25 or higher before pregnancy or in the first trimester
- You needed blood pressure medication within one week after giving birth
- You do not have a history of heart failure, genetic heart muscle disease, or severe kidney problems
