- You have type 2 diabetes but do not use insulin.
- Your last A1C (average blood sugar over 3 months) was between 8.0 and 12.0%.
- You are enrolled and ready to start the VA's VDOP diabetes program.
- You are willing to wear a continuous glucose monitor (a small sensor) on your body.
- Your diabetes medications have not changed in the last 3 months.
- You are not pregnant, planning pregnancy, on dialysis, or have certain other health conditions.
