Who can take part
- You must be at least 18 years old.
- You must be a family member (or someone close) caring for an older adult age 65+ who lives in a care home.
- You need to be able to read, write, and speak Cantonese or Mandarin.
- The older adult you care for must be considered 'prefrail' or 'frail' (score 3 or higher on the Clinical Frailty Scale).
- You must have a smartphone or other mobile device that can receive texts and use the internet.
- You should have had little or no prior conversations with the care home resident about end-of-life care wishes.
