  • You are 18 years old or older.
  • You live in the community (not a nursing home or hospital) in Maryland.
  • You have a physical or sensory disability.
  • You provide at least 10 hours of care each week for a family member or friend.
  • You do not have a terminal illness with less than a year to live and do not have severe memory or thinking problems.
