Who can take part
- You must have been in the ICU for at least 48 hours.
- You must be 65 years or older.
- You must be alive and ready to leave the hospital soon, expected to live at least 6 months.
- You must have a new disability that started during your hospital stay.
- You must plan to live in Australia for the next 3 months and go home (not to a facility) within 2 weeks of discharge.
- You need a regular doctor (GP) and must not already have a recovery navigator.
