  • Spent at least 48 hours in the ICU within the past year
  • Have at least one type of lasting problem from ICU stay: weakness (muscle or nerve damage), memory trouble, depression/anxiety, malnutrition, or bone weakness
  • Able to understand and sign consent to participate
  • Not currently confused or delirious (alert and oriented)
  • No severe dementia or thinking problems before the ICU stay
