Who can take part
- You are between 18 and 85 years old
- You have heart failure classified as NYHA class II or III (moderate symptoms)
- Your heart's pumping ability (left ventricular ejection fraction) is 40% or less
- You have an implanted defibrillator (ICD) or a special pacemaker (CRT-D) that is compatible with the SignalHF system from Biotronik, Boston Scientific, or Medtronic, and it has been in place for at least 30 days
- You are willing to be monitored remotely through the Implicity platform
- Your heart failure treatment follows European guidelines
