  • You are 18 years or older
  • You have a blood cancer that requires a stem cell transplant
  • You have at least 4 weeks before your scheduled transplant
  • You are considered 'pre-frail' or 'frail' based on a specialized frailty scale
  • You can do light daily activities (ECOG 0-2 or Karnofsky 60 or higher)
  • You have internet access for remote video sessions
