  • You are being discharged from the emergency department or an acute care unit.
  • You are currently taking a blood thinner (like warfarin, enoxaparin, dabigatran, rivaroxaban, apixaban, or edoxaban), either as a new or ongoing prescription.
  • You are part of the CSAPG healthcare area.
  • You are not being discharged from an outpatient clinic, home hospital service, intermediate care, or a residential facility.
