  • You have been diagnosed with coronary heart disease.
  • You are comfortable using a smartphone.
  • You do not have problems with vision, hearing, thinking, or movement.
  • You are able to attend in-person follow-up visits.
  • You do not have a severe illness like advanced heart failure, cancer, or end-stage kidney or liver disease.
  • You are willing to exercise and have no condition that would prevent it.
