Who can take part

  • You are 18 years or older
  • You have an upcoming or recent clinic visit (in person or by video)
  • You have fatty liver disease (seen on imaging) plus at least one heart or metabolic risk factor, or you have had high liver enzymes for more than 6 months, or you have prediabetes or diabetes, or you have 2 or more heart or metabolic risk factors
  • You have not had an organ transplant
  • You have not seen a liver specialist in the last 3 years
  • You do not have an active cancer diagnosis, alcohol-related conditions, or are pregnant or receiving palliative care
