Who can take part

  • You are 18 or older and have been diagnosed with heart failure (NYHA class II to IV, meaning you have some symptoms like shortness of breath or fatigue).
  • Your self-care is currently poor, as determined by a baseline assessment.
  • You get care at a participating heart failure clinic.
  • You have internet access and a device (like a smartphone, tablet, or computer) for video calls.
  • You have a primary informal caregiver (like a family member or friend) who is 18 or older, provides unpaid daily help, and is willing to join the study.
  • You have not had a heart attack or other acute cardiac event in the last 3 months, and you do not live in a nursing home or have severe memory problems that would make it hard to participate in video sessions.
