Who can take part

  • You are 21 years old or older.
  • You speak English, Chinese, or Malay.
  • You have stage C or D heart failure and have been told by your doctor that you have some trouble with physical activity (NYHA class 2 or higher).
  • Your doctor expects you to live at least 6 more months and you have been in the hospital for heart failure in the last 6 months.
  • You have a phone you can use for calls.
