  • You are a family member or close friend caring for someone with memory or thinking problems
  • The person you care for was seen in the emergency room and sent home (not admitted to the hospital)
  • You are 18 years old or older
  • You can read and understand English or Spanish
  • You have a mobile phone that can receive text messages
  • The person you care for lives at home, not in a nursing home or long-term care facility
