  • You were hospitalized and have admission notes that describe why you came in.
  • Your hospitalization involved symptoms or findings of a coronary syndrome (chest pain, heart attack, or reduced blood flow).
  • Your medical records clearly show one of four types: STEMI, NSTEMI, unstable angina, or stable coronary disease.
  • Your diagnosis was not unclear or missing important timing details.
