  • Be 18 years or older.
  • Be a patient of a family doctor (general practitioner) that is taking part in this study.
  • Have an advanced long-term condition, such as stage IV cancer, severe COPD, severe heart failure, or advanced kidney disease.
  • Not be in the middle of a medical crisis or needing emergency care right now.
