This study compares different ways to help people with long-term traumatic brain injury (TBI) manage their health care. If you have had a TBI for at least a year and see a brain injury doctor, you may be able to join—especially if you also have other health conditions or use multiple medications.

Who can take part
- You are at least 18 years old and live in the community (not in a facility).
- It has been at least one year since a traumatic brain injury (TBI) that caused you to lose consciousness.
- You are currently seeing a brain injury doctor who will refer you to the study.
- You can understand and communicate in English, and have a webcam and internet access.
- You have at least one of the following: 2 or more chronic health conditions, 5 or more medications, 3 or more ER visits in the past year, 1 or more hospital stays in the past year, or you see 2 or more specialists.
