- You are 18 years old or older
- You are Qatari or Arab
- You were admitted to the hospital for a heart attack (acute coronary syndrome) or acute heart failure
- You have diabetes or prediabetes, shown by a blood sugar test (HbA1C) of 5.7% or higher, or you take diabetes medication
