  • You are at least 18 years old
  • You had heart surgery (bypass, valve repair/replacement, aortic surgery, congenital heart repair, or heart device implant) at Beijing Anzhen Hospital
  • You are medically stable and ready to leave the hospital after surgery
  • You can use a smartphone app yourself, or have a family member who can help you use it
  • You are willing to participate, understand the study, and can complete 12 months of follow-up care
