Who can take part

  • You are 18 years or older
  • You have heart failure that causes some limits on activity (NYHA Class II or III)
  • You are stable enough to leave the hospital (no IV heart medications in the last 48 hours)
  • You have a smartphone (iPhone or Android) and know how to use it
  • You have a family member or friend who can help with the process
  • You are not expected to live less than 6 months, and do not have severe memory problems
