This study helps people with high blood pressure who struggle to afford food. Researchers will work with your regular doctor's office to understand how food insecurity affects your blood pressure and find ways to support you better.

Who can take part
- You are a patient at one of the participating health center clinics
- You are 20 years or older
- You have struggled to afford enough food in the past 12 months (screened using a simple two-question tool)
- Your doctor has diagnosed you with high blood pressure, and your recent reading was 130 mmHg or higher
- You have not had a prior heart attack, stroke, or other major heart/blood vessel problems
- You can speak, read, and understand English well enough to complete the study
