  • You have a VA medical record diagnosis of schizophrenia, schizoaffective disorder, or bipolar disorder
  • You received care at a VA site in the past 9 months
  • You score high on a risk assessment called CAN, meaning you're at higher risk for hospitalization or serious outcomes in the next year
  • You own and can use a smartphone during the study
