  • You have been diagnosed with type 2 diabetes (not type 1)
  • You currently see a doctor at one of the participating medical centers
  • You have a smartphone and reliable internet access
  • You are not currently pregnant
  • You are able and cleared by your doctor to make changes to diet and exercise
