- You are 18 years of age or older
- You live in either a rural or urban area
- You are either healthy, have a chronic disease (like diabetes or heart disease), are in addiction treatment with methadone or buprenorphine, or are currently incarcerated
- You do not have an active communicable (contagious) disease like COVID-19, tuberculosis, or hepatitis
