  • You are 45–75 years old
  • Diagnosed with Parkinson's disease 4–10 years ago
  • Your Parkinson's causes slowness of movement (bradykinesia) plus stiffness, tremor, or balance problems
  • You experience motor fluctuations (on/off periods) documented in a symptom diary
  • You've been on stable Parkinson's medications for at least 4 weeks
  • Levodopa medication helps your symptoms
