- You are between 50 and 74 years old
- You have been diagnosed with type 2 diabetes (not type 1)
- Your diabetes and other chronic condition medications have been stable for at least 4 weeks
- You have not had a serious diabetes emergency (like diabetic coma or severe low blood sugar) in the past 6 months
- You do not have severe heart disease, uncontrolled high blood pressure, or recent stroke or heart attack
