Who can take part

  • You have high blood pressure (readings consistently above 130/80 mmHg) or take medication for it
  • You also have at least one other chronic condition: high cholesterol, diabetes, kidney disease, or a history of heart or stroke disease
  • You can use a home blood pressure monitor or blood pressure cuff and share readings regularly
  • You do not have severe heart failure, end-stage kidney failure requiring dialysis, severe liver disease, or active cancer
  • You are not pregnant and do not plan to become pregnant during the study
