- You are scheduled for knee replacement surgery (one knee only) at a UPMC hospital
- You plan to do your recovery and physical therapy through UPMC Home Health and/or UPMC Rehab Institute after surgery
- You must actually have the knee replacement surgery as planned
- You must receive at least some rehabilitation care at UPMC facilities after your surgery
