  • You are 65 years old or older
  • You have or recently had cancer and may still be receiving treatment
  • You were recently hospitalized and have been discharged
  • You show signs of frailty (weakness or loss of function) at hospital discharge
  • You can safely participate in exercise with medical guidance
  • You live at home (not in a facility like a nursing home or assisted living)
