This study tests whether personalized meal plans designed by doctors can help people with type 2 diabetes and high blood pressure improve their heart health. You'll receive specially prepared meals for 8 months and answer questions about your health.

Who can take part
- You have type 2 diabetes (confirmed by a doctor, blood test, or diabetes medication)
- You have high blood pressure or elevated blood pressure (confirmed by readings, doctor diagnosis, or blood pressure medication)
- You are already enrolled in the parent MTM2MTG study
- You speak English or Spanish fluently
- You can understand study instructions and give informed consent
- You do not have cancer, severe mental illness, or conditions that would make meal participation unsafe (like dialysis or substance use disorder)
