  • You have a lumbar disc herniation (a slipped disc in your lower back) and need single-level fusion surgery
  • You have never had lumbar spine surgery before
  • You have reliable WiFi internet at home
  • Your surgery will be performed by the same surgical team
  • You are able and willing to follow the study plan and exercise guidance
