Who can take part
- You live in Indiana and get health insurance through a participating employer
- You are between 18 and 64 years old
- You have a BMI (body mass index) of 30 or higher, OR a BMI of 25+ with a weight-related condition like high blood pressure or diabetes
- You've had continuous health insurance coverage for at least 6 months before joining the study
- You haven't had weight-loss surgery or been treated with certain diabetes or weight-loss medications before
