- Age 18 or older
- Rectal cancer confirmed by biopsy, located 5–15 cm from the anal opening
- Cancer is stage T2 or T3 with no spread to distant organs, and surgeons agree it can be removed with curative intent
- Your cancer has at least one high-risk feature (lymph node involvement, tumor close to the tissue boundary, or blood vessel invasion)
- Normal or low microsatellite instability status (a genetic marker) on pathology testing
- Adequate heart, kidney, and liver function based on recent blood tests
