  • You gave birth at Boston Medical Center in the last 2 months.
  • You had high blood pressure or a related condition (like preeclampsia) during your pregnancy.
  • You are currently taking a blood pressure medication.
  • You plan to continue getting your care at Boston Medical Center.
  • You do not have a heart condition and are not planning to become pregnant in the next 3 months.
  • You are not planning to move out of Massachusetts within the next year.
