- You are a parent or legal guardian of a child under 17.5 years old.
- Your child had a cardiac arrest that lasted at least 2 minutes in the past 7 days.
- Your child is still in the pediatric or cardiac intensive care unit (PICU or CICU).
- Your child is expected to be discharged from the hospital within 30 days and survive at least 3 months after leaving.
- You speak English.
