- Your baby must be full-term (born between 37 and 42 weeks).
- Your baby's birth weight must be between 5.5 and 9.9 pounds (2500-4500 grams).
- Your baby must be between 4 weeks and 5 months old at the start.

- Your baby must have a diagnosed cow's milk protein allergy, either immediate or delayed symptoms.
