Who can take part

  • You are 18 years old or older
  • You are coming for a first visit at an outpatient physical therapy clinic for neck, mid-back, or low back pain (may include pain traveling down your arm or leg)
  • You can read and understand study questionnaires in English
  • You have not had spine surgery in the last 3 months
  • You do not have a serious spine condition like cancer, infection, or a recent fracture currently being treated by a doctor
