  • You are eligible for bariatric (weight-loss) surgery based on common NIH guidelines
  • Your BMI is above 40 but below 55
  • You’ve had obesity for more than 5 years and tried a supervised weight-loss program without enough success
  • You understand how surgery helps with weight loss and agree to follow diet, medical plans, and follow-up visits
  • You understand the goal is not to reach an “ideal” weight
  • You can sign consent, and you can avoid pregnancy for 1 year after surgery (for women)
