- Your hearing test shows significant loss in the “active” ear, without meaningful improvement after steroids
- You tried steroid treatment (by mouth or into the ear) but either did not improve or could not tolerate steroids, and you have finished steroids
- Imaging does not show certain inner-ear problems (like a nerve tumor or inner-ear shape issues)
- You are not on immune-suppressing medicines like methotrexate or anti-TNF drugs during the study
