Who can take part
- You’re an adult and had your first confirmed stroke (either bleeding or clot),
- You’re currently being treated at the participating rehab center and plan to go home directly from there,
- Your disability score at discharge planning is mild to moderate (modified Rankin 1–3),
- You live in the Rhône department and can participate by completing questionnaires (or with caregiver help for speech/aphasia),
- You can give written consent (and you must have social health insurance).
