  • You have been diagnosed with a stroke
  • You are finishing or recently finished hospital rehabilitation (within the last 3 weeks)
  • You lived in your own home before your stroke
  • You are going home to your own residence or a family member's home (not a nursing home or care facility)
  • You live in Toronto or Ottawa
