- You have had a stroke that is confirmed by a doctor
- You are currently in or have recently finished inpatient rehabilitation within the last 3 weeks
- You lived at home before your stroke, not in a nursing home
- You are going directly home (to your own home or a family member's home) after leaving the hospital
- You live in Toronto or Ottawa and can communicate in English
- You are able to understand and agree to participate (no severe memory or thinking problems that prevent consent)
