Who can take part

  • You can sign consent and agree to follow study visits for the full study
  • You have COPD or heart failure (and it’s the main condition being studied)
  • You also have at least two other long-term conditions such as diabetes, high blood pressure, kidney disease stage 3b or worse, anemia, liver disease, or obesity
  • You do not currently smoke cigarettes
  • You can walk at least 300 meters in the 6-minute walk test
  • You can safely do regular physical activity for 24 weeks (no major heart/lung limits)
