Who can take part

  • You have chronic heart failure with reduced ejection fraction (HFrEF) or mildly reduced ejection fraction (HFmrEF), confirmed by an echo in the last 6 months.
  • You are 18 years or older.
  • You feel short of breath or tired with mild activity (NYHA class II) or with less than usual activity (class III).
  • You have not had any worsening heart failure symptoms in the last month.
  • You have not had a heart attack, heart surgery, or a new heart device in the last 3 months.
  • You (or a caregiver) have a smartphone or internet and can use a simple app like WhatsApp.
