- You have been diagnosed with acute lymphoblastic leukemia (ALL) or acute myeloid leukemia (AML)
- Your birth cord blood was stored, and this is recorded in a study registry
- Pre-treatment leukemia samples from your original diagnosis are available to be requested for testing
- You joined a Children’s Oncology Group (COG) research program called APEC14B1 with permission for future contact
- Your original diagnosis was at age 25 or younger
- You can read and understand English or Spanish, and you (and a parent/guardian if needed) can give consent
