This trial tests whether a fecal microbiome transplant (FMT) can help your body respond better to anti-PD-1 immunotherapy in advanced skin melanoma. It is for people whose melanoma has progressed despite anti-PD-1 treatment.

Who can take part
- You are age 18 or older with advanced skin melanoma (stage III or IV).
- If you have stage IV, it must be limited to M1a or M1b (not M1c or M1d).
- Your melanoma has grown despite anti-PD-1 treatment, with at least a 20% increase on two scans at least 4 weeks apart.
- You must have at least one measurable tumor area on scans and be able to swallow the required bowel cleansing for the procedure.
- You need good general health for the study: life expectancy over 3 months and good blood/liver/kidney test results (for example, LDH at or below 1× the lab upper limit).
- You must not have certain melanoma types (acral, uveal, mucosal, or unknown primary) and must not have had other melanoma treatments besides anti-PD-1 so far.
