  • You are age 16 or older
  • You had an angiogram at BHC (your heart scan happened there)
  • You had a revascularisation procedure (a treatment to restore/improve blood flow)
  • You were referred and seen by the Interventional Cardiology team (or meet the registry referral process)
  • You must not be younger than 16
