Who can take part

  • You have a diagnosed heart or blood vessel condition such as ischemic heart disease, prior heart attack, heart failure, or stroke/TIA.
  • You may qualify if you were newly diagnosed with a first heart attack by a community doctor and were hospitalized for at least 3 days within a 30-day window before diagnosis.
  • You qualify if you have had procedures like bypass surgery (CABG) or a stent/angioplasty (PCI) in the past, based on medical billing records (CPT codes).
  • You must have medical records that do not suggest you have no coronary heart disease (CHD) according to your main doctor.
  • You should not be someone who was labeled with coronary heart disease before the year 2000 with no continued evidence afterward.
