Who can take part

  • You’re age 65 or older and receive care from a local community health center
  • You have type 2 diabetes (not type 1), confirmed by blood sugar testing
  • You’re also at higher risk for heart problems and/or have chronic kidney disease
  • Your kidney function is not too low (eGFR is 30 or higher)
  • You can be followed for about 36 months and are willing to sign consent
