Who can take part
- You are 19 years or older.
- You have type 2 diabetes (not type 1).
- Your A1c (recent blood sugar average) is between 7.0% and 10.0% and you take 1–3 oral diabetes medicines.
- You have a history of heart disease, stroke, or related blood vessel conditions, OR you have at least two of these risk factors: smoking, high blood pressure, low good cholesterol, family history of early heart disease, or obesity.
- You do not have severe heart failure, have not been in the hospital for a heart attack/stroke in the last 3 months, and do not have active cancer or very poor kidney function.
